Crash Reports

The Ohio State Highway Patrol maintains complete crash reports and crash photographs from crashes investigated by the Ohio State Highway Patrol only. The crash reports and photographs are retained for five years.

Please wait a minimum of seven business days from the date of the crash before attempting to order a crash report to ensure the availability of the completed initial report. Also, please be aware some crashes require additional investigation beyond the initial report. Additional information resulting from these investigations may not be available until such investigations are complete. If you have any questions about obtaining a crash report or are unable to view a report you have purchased, please send an e-mail to oshpcrashreports@dps.ohio.gov, or contact the Central Records Unit via mail, telephone or fax as indicated below.

Cost & Delivery
  • Crash reports purchased online will be available to you once your purchase is confirmed. The report will then be accessible by you for up to 72 hours for printing or downloading.
  • Crash photographs purchased online will be either placed on a DVD and sent in the mail, or downloaded electronically. If you choose to receive the photos on DVD, Please allow 4-6 weeks for delivery. If you choose to receive photographs electronically, please allow 24-48 hours to receive the photograph link. Link expires in 14 days after it has been sent. Please see below for Report and Photo price structure.
  • Visa, MasterCard, Discover and American Express are currently accepted. A transaction fee will be added for each credit card transaction.

To order crash reports and/or photos online you must have a web browser capable of running Javascript, a valid credit card, and a viewer that is able to view PDF.

CRASH REPORT REDACTIONS

Certain private information is routinely redacted from traffic crash reports as they are not considered public records under the following statutes:

  • Driver’s license numbers – R.C. 149.43(A)(1)(dd)
  • Dedicated public service worker residential and familial information – R.C. 149.43(A)(1)(p), (A)(7), and (A)(8)
  • Confidential law enforcement investigatory records – R.C. 149.43(A)(1)(h) and (A)(2); R.C. 5502.12
  • Telephone numbers of parties to a traffic crash – R.C. 149.43(A)(1)(mm)
  • Personal information of a juvenile involved in a school vehicle traffic crash – R.C. 149.43(A)(1)(gg) (See below for how to obtain a report if you are the juvenile’s parent or guardian)
SCHOOL VEHICLE TRAFFIC CRASH REPORT

The OHP 0413 Request for Records must be completed by a parent or guardian requesting student traffic crash records kept by the Ohio Department of Public Safety (ODPS) and the Ohio State Highway Patrol (OSHP).

This form must be completed by parents and guardians of an individual:

(1) Who is less than eighteen years of age, and

(2) Who was in a traffic crash that involved a school vehicle, e.g., was an occupant on a school bus.

*NOTE: You must complete the purchase process to receive a copy of the crash report. The requestor will receive a redacted copy of the crash report after their purchase has been processed. Once the OHP 0413 Request for Records is received and verified, another copy of the crash report with the information from the requestor’s child ONLY will be sent to the requestor at no additional cost.

Public Records

The Central Records Unit is responsible for maintaining records for the Ohio Highway Patrol and the Ohio Investigative Unit. To request a copy of criminal investigation or to make a request for other public records under Ohio Revised Code 149.43, please go back to the previous page to submit your request.
